What happened?

Two questions here. Nothing you answer is shared with any insurer.

Type of incident

An approximate date is fine. Arizona deadlines run from this date.

Confidential. No fee unless we win.

The fee, stated in numbers

Most firms describe the contingency fee in a paragraph designed not to be read. Here it is as arithmetic, on a hypothetical $300,000 recovery resolved before filing.

Illustrative fee breakdown on a $300,000 pre-suit recovery
Gross recovery$300,000
Attorney fee at 33.3 percent$99,900
Case expenses, typical pre-suit$4,200
Medical liens, after negotiation$38,500
To you$157,400

Illustration only. Lien negotiation is a real part of the work and routinely returns more to the client than the difference between one firm's fee and another's.

Before you send it

What exactly does the fee work out to?

One third of the recovery if the case resolves before a lawsuit is filed, and 40 percent after filing. Case expenses such as expert fees, records and depositions come out of the recovery rather than out of your pocket. If we recover nothing, you owe nothing, including expenses.

Does sending this form commit me to anything?

No. It is a review, not a retainer. Nothing here creates an attorney-client relationship and you are free to take the information and do nothing with it, or take it to another firm.

What if I already have a lawyer?

You are allowed to change lawyers at any point, and it usually costs you nothing extra because the two firms divide a single fee. If you are unhappy with how your case is being handled, a second opinion is worth twenty minutes.
